The following guidelines help ensure successful diagnosis and treatment.

Below is our listed Preoperative Exam and Lab tests which are required prior to a scheduled surgery.

PREOPERATIVE BLOOD TESTS
The following blood tests are required before we schedule a tubal reversal. These blood tests can be ordered by your local physician and are good for 30 days.

• Complete Blood Count + Chem-25 + PT, PTT
• HIV Screen
• Hepatitis A, B, C Screen

Additional lab tests may be required based on your medical history. Please request your doctor fax the test results to us at 773.522.3263. Preoperative Lab Tests are due in our office at least two weeks prior to the surgery date.

WOMEN 40 YEARS AND OLDER FSH TEST (OPTIONAL)
Patients older than 40 years may want to schedule a cycle day 3 FSH assay to assess their ovarian function.

SEMEN ANALYSIS (STRONGLY RECOMMENDED)
Partners should order a semen analysis by their local physician.
